You don't need a printout of it. You only need the BSR code of the bank, date of payment, Assessment Year, Nature of Payment, bank name and branch where you have paid.
See your account statement if it is having all those details or not. Otherwise contact your banks banks helpline number. If you fails to obtain the said details, then register yourself with NSDL to view your tax credit with a payment about Rs.20/- as fee for registration. In that you can fill all the taxes paid by you in detail.

more than that you can download the commoan application form from most websites . fill it and sent it by post with a first cheque . the next cheques can be done by submitting the ECS form along with the first cheque.
